Job Summary
This position is primarily responsible for facilitating the admissions of patients to hospice services at Seasons Hospice during the weekend. This position will also make patient visits and assist with staffing at the hospice house as time allows.
Job Specifications
  • Education and Training – Graduate of an associate’s, diploma, or baccalaureate nursing program
  • License or Certification – Current RN license by applicable state requirements; current BLS for Health Care Providers or will obtain within 3 months of start date; valid driver’s license with acceptable driving record per policy
  • Experience -Two years RN experience within the last five years preferred
  • Additional skills - Daily access to a reliable and insured vehicle. Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, prior experience with electronic medical record systems and belief in the hospice concept
Job Responsibilities
  • Assume initial responsibility of patient/family needs and complete admission paperwork.
  • Determine patient’s appropriateness for hospice services consistent with Seasons Hospice admission criteria, in conjunction with the patient’s physician, the hospice medical director/hospice physician, and other team members.
  • Obtain data on physical, psychological, social and spiritual factors that may influence patient health status and incorporate that into the plan of care.
  • Document problems, nursing assessments, appropriate goals, coordination of care, care interventions and patient and family outcomes achieved from intervention and care provided.
  • Assess all aspects of the patient's pain and develop an individualized pain management plan.
  • Anticipate, prevent and treat undesirable symptoms or secondary symptoms.
  • Initiate communication with physicians and other staff members to coordinate optimal care and use of resources for the patient/family.
  • Maintain documentation relating to care coordination including phone calls, communication, visits, and other pertinent clinical information.
  • Teach appropriate interventions, medications, effects and side effects to patient, caregiver, volunteers and other as appropriate.
  • Communicate with various disciplines, regarding unique or potentially challenging patient and/or family situations.
  • Make patient visits to wherever the patient calls home; as indicated by need.
  • Work in the office assisting with medical records and order processing along with other duties as needed.
  • HIS Admission and Discharge data submission-back up response.
  • Assist at Hospice House as needed and as time allows.
  • Put storeroom supplies away PRN, clean equipment and general organization of supplies/equipment.
  • Implement care to achieve outcomes of comfortable and safe dying, self-determined life closure and effective grieving for patient and family.
  • Responsible for maintenance of electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards.
  • Identify and address comfort care needs of patient.
  • Support, instruct and educate the patient, family, and caregiver.
  • Maintain the dignity of the dying patient.
  • Support the patients and family's unique spiritual and cultural beliefs.
  • Provide holistic, family-centered care across treatment settings to improve the quality of life.
  • Consult and collaborate with the interdisciplinary team (IDT) and others involved in the patients care.
  • Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.

  • Maintain absolute confidentiality of all information pertaining to patients, families, and staff. Adhere to all Health Insurance Portability and Accountability Act (HIPAA) rules and regulations.
  • Display a positive and professional image and attitude in all relationships with patients, families, peers and in the community.
  • Serve as an ambassador for Seasons Hospice at all times.
  • Communicate clearly, concisely and accurately, creating a work environment that provides growth and job satisfaction for self and others.
  • Provide verbal and/or written performance reviews for assigned staff members, including peer and self evaluation.
  • Work effectively in a team environment.
  • Demonstrate ongoing responsibility for self education.
  • Maintain patient and personal safety regarding occupational health and safety.
  • Assume responsibility for the promotion of the hospice concept in the community and with other agencies and professionals.
  • Actively participate in committees within the agency.
  • Attend scheduled department meetings and agency meetings on a regular basis.
  • Participate in the safety program and promote a safe work environment.
  • Participate in the Quality Assessment Performance Improvement Program (QAPI); as requested.
  • Participate in orientation, training and education of new employees and volunteers; as requested.
  • Participate in orientation and supervision for students seeking experience in hospice care; as requested.
  • Perform other duties as requested and/or assigned.

May spend portion of time working on a computer.  May occasionally work longer than 8 hours/day. Must be able to tolerate some variability in work hours; must have ability to be flexible with work schedule. Must be able to work independently and in a team environment.
