Statement of Position/Position Summary
This position will assume overall responsibility for the implementation, delivery, coordination and supervision of hospice services provided in the homecare setting and at the hospice house.
Qualifications and Education requirements
  • Graduate of an accredited school of nursing: Bachelor’s degree in nursing required, Masters Degree preferred.
  • Registered nurse with current licensure to practice professional nursing in the state of MN.
  • Minimum of eight (8) years of clinical experience including five (5) years in a managerial role in a healthcare environment; preferably a hospice environment.
  • Demonstrated skills in administrative performance, including expertise in written/oral communication, supervision of staff, documentation requirements, service coordination and case management.
  • Comprehensive knowledge of nursing practice, applicable rules and regulations and industry standards pertaining to hospice care.
  • Computer proficiency and a working knowledge of Microsoft programs required.
  • Current CPR certification.
  • Valid MN Drivers License and daily access to a reliable and insured vehicle.
  • Belief in the hospice concept.
Position Role and Responsibilities
  • Serve as a member of the leadership team for this Agency.
  • Assume overall responsibility for oversight and management of the clinical services provided by the interdisciplinary team (IDT) within the homecare and hospice house setting.
  • Assist in the development of agency goals. Develops, recommends, and administers Agency policies and procedures.
  • Supervise all clinical staff including RN, LPN, Hospice Aide, TMA, Social Worker, Chaplain, Educator, Massage Therapist, Music Therapist, Hospice House Manager and Medical Office Assistant.
  • Provide information for inclusion in written performance evaluations for all direct report staff and submits written performance evaluations in accordance with policy requirements.
  • In conjunction with the Educator role, identifies the need for staff education and assists in the development of in-service programs for members of the IDT team.
  • Provides leadership and counsel to clinical personnel to enhance staff development.
  • Participate and provide leadership during IDT meetings and other meetings/committees related to clinical practice.
  • Participate in events of Seasons Hospice as a member of the leadership team. (i.e., Gala, Memories of the Heart, etc.)
  • Conduct supervisory visits to allow for observation of clinical performance of assigned team members.
  • Supervise the maintenance of medical records assuring accuracy, completeness and compliance with licensing regulations and legal and ethical standards.
  • Assist with oversight and responsibility for the Quality Assessment and Performance Improvement (QAPI) program, to include:
    • Development of annual QAPI plan
    • Collect and assess data regarding patient services and outcomes according to QAPI plan.
    • Compile and submit data from CHAPS survey according to CMS guidelines.
  • Participate in staff selection and orientation of new personnel as appropriate.
  • Serve as the agency HIPAA compliance officer.
  • Receive, investigate and make recommendations regarding patient complaints in accordance with Seasons Hospice policy and procedure.
  • Serve as a facilitator (or assign a designee) for nursing, medical and other students seeking learning opportunities at Seasons Hospice.
  • Communicate all pertinent clinical and administrative information to the Executive Director and other members of the team as appropriate.
  • Participate in community and professional education regarding end-of-life care and hospice services.
  • Serve as a liaison for community health partners including assisted living facilities, long term care facilities, adult foster care homes and other living facilities in which Seasons Hospice staff provide services.
  • In the absence of the Executive Director, the Director of Clinical Services serves as the Privacy Official and is responsible for decision making and daily operations.

  • Maintain absolute confidentiality of all information pertaining to patients, families, and staff and adhere to all HIPAA rules.
  • Display a positive and professional image and attitude in all relationships with patients, families, peers and in the community.
  • Serve as an ambassador for Seasons Hospice at all times.
  • Provide verbal and written performance evaluations for assigned staff members, including peer and self evaluation.
  • Demonstrate ongoing responsibility for self education.
  • Assume responsibility for the promotion of the hospice concept in the community and with other agencies and professionals.
  • Participate in the Quality Assessment Performance Improvement Program (QAPI), as requested.
  • Maintains patient and personal safety regarding occupational health and safety.
  • Participate in safety program, as requested:
    • Participate in employee job safety analysis to eliminate hazards and prevent accidents.
    • Cooperate to investigate employee injuries.
    • Promote and support safety awareness and implement safety operations.
    • Encourage staff to make suggestions on safety practices.
  • Participate in orientation of new employees.
  • Participate in volunteer orientations, training and education as requested.
  • Participate in orientation and supervision for students seeking experience in hospice care. Attend regularly scheduled department meetings and agency meetings.
  • Perform other related duties as assigned.
