Job Summary
This position will plan and provide music therapy consultations and services to patients in accordance with the Interdisciplinary Team (IDT) Plan of Care, initiating music therapy programs and instructing other staff members and family members on these programs. Serve patients and families in their homes, hospitals, nursing homes, and all other facilities that patients consider their home.
Job Specifications
  • Education and Training – Bachelor’s degree from an approved college of Music Therapy.
  • License or Certification – Music Therapist- Board Certified (MT-BC); valid driver’s license with acceptable driving record per policy
  • Experience -Two years experience within the last five years preferred; health care experience preferred.
  • Additional skills - Daily access to a reliable and insured vehicle. Must have strong written and verbal communication skills, strong organizational skills, accurate and attentive to details, proficient keyboarding skills, prior experience with electronic medical record systems and belief in the hospice concept.
Job Responsibilities
  • Provide music therapy consultations and services to patient and/or family in accordance with the interdisciplinary plan of care.
  • Provide music therapy to patients and families in their homes, hospital, nursing homes, and any other facility the patient calls home.
  • Prepare clear and accurate documentation of patient’s music therapy program for the medical record.
  • Present in-service programs for staff, volunteers, and community agencies demonstrating the effects of music therapy within hospice settings.
  • Participate in the interdisciplinary team (IDT) meeting to establish, review and revise the plan of care for each patient, including exchange of information, review of problems, and sharing of professional support.
  • Maintain patient and personal safety.
  • Participate in volunteer orientation, training and education as requested.
  • Participate in orientation of new employees.
  • Participate in orientation and supervision for students seeking experience in hospice care.
  • Responsible for maintenance of electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards.
  • Identify and address comfort care needs.
  • Support, instruct and educate the patient, family, and caregiver regarding music therapy.
  • Maintain the dignity of the dying patient.
  • Support the patients and family's unique spiritual and cultural beliefs.
  • Provide holistic, family-centered care across treatment settings to improve the quality of life.
  • Consult and collaborate with the interdisciplinary team and others involved in the patients care.
  • Provide care consistent with National Hospice and Palliative Care Organization standards of practice for hospice programs.

  • Maintain absolute confidentiality of all information pertaining to patients, families, and staff. Adhere to all Health Insurance Portability and Accountability Act (HIPAA) rules and regulations.

  • Serve as an ambassador for Seasons Hospice at all times.
  • Communicate clearly, concisely and accurately, creating a work environment that provides growth and job satisfaction for self and others.
  • Provide verbal and/or written performance reviews for assigned staff members, including peer and self evaluation.
  • Work effectively in a team environment.
  • Demonstrate ongoing responsibility for self education.
  • Maintain patient and personal safety regarding occupational health and safety.
  • Assume responsibility for the promotion of the hospice concept in the community and with other agencies and professionals.
  • Actively participate in committees within the agency.
  • Attend scheduled department meetings and agency meetings on a regular basis.
  • Participate in the safety program and promote a safe work environment.
  • Participate in the Quality Assessment Performance Improvement Program (QAPI); as requested.
  • Participate in orientation, training and education of new employees and volunteers; as requested.
  • Participate in orientation and supervision for students seeking experience in hospice care; as requested.
  • Perform other duties as requested and/or assigned.

May spend portion of time working on a computer.  May occasionally work longer than 8 hours/day. Must be able to tolerate some variability in work hours; must have ability to be flexible with work schedule. Must be able to work independently and in a team environment.
