“Memories of the Heart” will be held on Tuesday, December 3, 2019 at Christ United Methodist Church (400 Fifth Avenue SW, Rochester, MN). This year we will be hosting our reception prior to the service. Open reception begins at 6:00 p.m. followed by the service at 7:00 p.m. For the past twenty years, Seasons Hospice has provided this special way to remember loves ones who have died. The service includes music, reflection, and the reading of names of those loved ones being remembered. Please join us for this event which is open to all who wish to remember someone. A memorial board will be available if you choose to bring a photo of your loved one.
Your loved one and all those served by Seasons Hospice this past year will be included in the reading list. If you prefer your loved one’s name not be read, please contact us. You may use the request form below to let us know if there are other names you would like to add to the reading list. Please feel free to share this form and event with others.

Remembering and Celebrating our Loved Ones

Seasons Hospice will be at the Community Volunteer Fair from 4:00-7:00 p.m. that is held at 125 Live.
Seasons Hospice’s Volunteer Services and pet therapy teams will be at Rochesterfest’s “The Magic of Service” from 11:00 a.m. to 2:00 p.m. on June 26th at the Rochesterfest grounds located at Soldiers Field. Soldiers Field is located directly behind Soldiers Field Veterans Memorial: 300 7th St SW, Rochester, MN 55902.

Seasons Hospice’s pet therapy teams will be at Rochesterfest’s “The Magic of Pets” from 11:00 a.m. to 2:00 p.m. on June 24th at the Rochesterfest grounds located at Soldiers Field.
Seasons Hospice will have a team walking at Relay for Life on June 21st. The walk will be held at RCTC from 5 p.m. until midnight. While at UMR, students are encouraged to engage in the Rochester community. Volunteer opportunities can be a great way for students to explore interests and contribute to an organization. Located in the University Square room 417.
Take the first step to a world without Alzheimer's
This event begins with an inspirational opening ceremony that pays tribute to our nation, the men and women of our military, and our cherished freedoms. The ceremony includes the presentation of colors, a wreath laying by Gold Star families, and singing of God Bless America and our National Anthem.
Following the opening ceremony golfers tee off with a shot-gun start for a 18-hole scramble tournament. Lunch is served prior to the event in the Clubhouse. There are opportunities to buy-in to on-course contests, purchase raffle tickets with prizes awarded during dinner, and participate in a silent auction.
Following the tournament, golfers and non-golfers are invited to join us for a cocktail reception and evening dinner program.
The public is welcome to the opening ceremony, lunch is available at $15.00 starting at 11:00am Followed by the Opening Ceremony- Honoring the Families of the Fallen at 12:00 pm.
A time to walk the beautiful Seasons Hospice House grounds and visit the Circle of Remembrance Memorial. Come enjoy reflective music, refreshments, and interact with staff and volunteers as you remember and celebrate your loved ones.

Come and learn about the new card and numbers. The Centers for Medicare & Medicaid Services (CMS) is fighting Medicare fraudulent activity by removing the SSN, patient’s sex, and the patient signature from the current Medicare card. A new card with a new Medicare number known as the Medicare Beneficiary Identifier (MBI) will be mailed to all Medicare beneficiaries. The overall mailing process will begin in April 2018. Hosted by Nikki Gruis Diekmann, Director of Quality & Compliance from Seasons Hospice to understand how to prepare.
